Wernicke's Encephalopathy Associated with Abnormal Liver Function in Hyperemesis Gravidarum


Abstract
A 33-year-old woman developed wernicke's encephalopathy in the 19th week of her pregnancy after 12 weeks of nausea and vomiting accompanied by moderately abnormal liver function land weight loss of 15kg. Although the patient did not recover
completely
at term of her pregnancy, she delivered a healthy fetus by cesarean section. Review of the previous literatures published during the last 25 years revealed 15 cases of Wernicke's encephalopathy complicating hyperemesis gravidarum. All of them
vomited
for at least 4 weeks. Rate of the patients with abnormal liver function was much higher than that reported in previous series of patients with hyperemesis gravidarum. Therefore parenteral thiamine supplementation may be essential to patients with
persistent hyperemesis gravidarum until an adequate diet is resumed, especially to those with abnormal liver function. And it is possible that the dysfunction of the liver plays a certain role in the development of Wernicke's encephalophathy in
hyperemesis gravidarum.
